Allergies

Evergreen School must be aware of any allergies and any allergy-specific treatment or medication.  Any medication must be accompanied by the State-required Medication Administration Authorization form available in the School office or on our website. Epi-pens require a similar form, available in the school office.  Any health emergency procedures must be outlined, in writing, by you and reviewed, step by step, with the Head of School and your child’s teacher.  They are also reviewed by our Delegating Nurse and our trained Medication Technician. Changes in a child’s condition must be reported promptly. 

A list of children with their specific allergies or chronic health conditions is confidentially kept by each teacher and inside a cabinet in the School office.
